Our client went to an emergency room due to constipation.  He then followed up with his family doctor and a pain doctor,

who failed to refer his to a gastroenterologist for care.  The failed refer ultimately resulted in his colon bursting, causing him

to have emergency surgery.  He lost over 50% of his colon and had to wear a colostomy bag for the rest of his life.
